Medical Exemption
An employee must provide a qualifying medical professional’s confirmation that they are currently exempt from receiving the vaccine using the COVID-19 Vaccine Medical Exemption form. If that form is completed and signed by an authorized medical professional, the employee will be eligible for a vaccine exemption. The types of medical professionals qualified to complete the form are listed on the form.

Religious Exemption
You will need to fill out and submit the Request for Religious Exemption to COVID-19 Vaccine form to the HR Benefits Department for review. If your religious exemption is approved, you will be eligible for a vaccine exemption. Note: Religious beliefs include theistic beliefs and “moral or ethical beliefs as to what is right and wrong which are sincerely held with the strength of traditional religious views.” Social, political or economic philosophies, as well as mere personal preferences, standing alone are not religious beliefs.
