Return to Work Process for Local S6 Union

Attendance at Work – Medical Absences

  • All LS6 employees are required to present to the Employee Health Department when returning to work following a medical absence of five (5) or more days and must provide the appropriate Return to Work Certification / Cardiac Clearance forms (Appendix 1 and 2 respectively).
    • Appropriate documentation from Health Care Provider should include:
      • Date/Employee Name/DOB/Badge Number
      • Dates of absence
      • Note of most recent date that provider saw employee
      • Return to work date
      • Acknowledgement of provider review of employee’s PTA
      • Confirmation of employee’s return to work with or without restrictions
      • Indication of narcotic/scheduled medication use with name/dose
      • Signature of Physician/Nurse Practitioner/Physician’s Assistant
  • Completed Return to Work Certification / Cardiac Clearance forms must be returned to BIW Employee Health Department via one of the following methods:
    • Fax: (207) 442-3386
    • Employee may bring completed documents in person
  • Any LS6 employee returning to work from an absence of less than five (5) days can present directly to Craft Administration with appropriate documentation for period of absence. Return to Work Certification / Cardiac Clearance forms are not required.
